Cost-Effectiveness Analysis of Introducing the Zoster Vaccine into National Immunization Program (NIP) of the Republic of Korea

Abstract
To evaluate the feasibility of introducing the zoster vaccine (ZV) to the Korean national immunization program (NIP), data on the disease burden of herpes zoster (HZ) and cost-effectiveness analysis (CEA) are needed. This study was conducted due to the lack of previous research in Korea on this topic. The epidemiologic characteristics and disease burden of HZ were established by literature review and data analysis of the Korea Health Insurance Review & Assessment Service (HIRA). A model was developed for the CEA of the ZV into the Korean NIP. The incidence and prevalence of HZ or postherpetic neuralgia (PHN) had been increasing year by year. Based on the analysis of the disease burden in Korea using the HIRA database, the number of new patients with HZ was found to be approximately 530,000 annually and 57,715 patients were accompanied by PHN. As of 2013, the socioeconomic cost due to HZ was estimated as 239,857,899,437 Korean Won (KRW) for all age groups and 164,710,507,719 KRW for ages 50 and over. The CEA of introducing the ZVL into the Korean NIP for ages 65 and over was found to be cost-effective; Incremental Cost-Effectiveness Ratio (ICER) 22,823,984 KRW/QALY, Willingness to Pay (WTP) 30,500,000 KRW). In conclusion, the introduction of ZVL into the NIP for ages 65 and over was found to be cost-effective in Korea. However, these findings alone cannot determine the introduction of the ZV, and further consideration of various factors should be considered such as the purpose of the NIP, the characteristics of adult vaccination, available finances, feasibility, and priority among candidate vaccines.
